Skilled Home Health

All skilled home health services are provided under physicians’ orders, and our staff maintains regular communication with your physician. Our services include:

• Nursing visits for dressing changes, catheter care, or progress assessments
• Health teaching for chronic illness management
• Intravenous medication administration
• Diabetes management counseling and monitoring
• Nutritional consultation
• Physical, occupational, and speech rehabilitation therapy
• Personal care for bathing, dressing, exercise, and other activities of daily living
• Medical social service assessment and referral to community agencies.

Home visits are scheduled at your convenience, according to your medical plan of care. Visits may be scheduled daily, weekly, or less often, depending on your needs. We have a registered nurse on call 24 hours a day/7 days a week for assistance after hours and on weekends.

Our expert home health personnel can teach you and your caregivers how to manage each aspect of your medical needs.
